This is the alignment of service resources that give people access to the full benefits of community living and ensure they receive services in a way that may help them achieve individual goals. It incorporates both Persian-Centered Thinking and Person-Centered Planning

Community Living

•Working with your regional center to make sure the current agreement
contains all ofthe elements of the Final Rule
•Model/sample agreements can be developed or provided to agencies
•Agreements should be signed and dated
People should be given a copy of the lease agreement and they
should be explained and
understood by residents.
